The ODA Health Insurance Plans are available to individual members and their families, as well as employer-employee groups of 2 or more.

The ODA Health Insurance Plans use PPO Oklahoma and Preferred Community Choice for Preferred Provider Networks. You have the freedom to choose any provider, but can receive optimal benefits by using the providers in the network of your choice. When you are outside of Oklahoma, you also have access to Physician Health Care Systems (PHCS) Healthy Directions Network.

 

High Deductible Health Plans

High Deductible Health Plan qualifies for use with a Health Savings Account (HSA).

The high deductible health plan (HDHP) rates are approximately 50% lower than the $1000 Deductible PPO Option.

Member Choice 100 - Under these HDHPs, eligible charges are applied to your deductible, then paid at 100% in-Network and 80% non-Network until yo meet your out-of-pocket maximum. Currently, the deductible choices for Member Choice 100 are:

Member Choice 80 - Under these HDHPs, eligible charges are applied to your deductible, then paid at 80% in-Network, and 60% non-Network, until you meet your ou-of-pocket maximum. Currently, the deductible choices for Member Choice 80 are:

$1000, $2500 and $7500 Deductible PPO Options

The $1000, $2500 and $7500 deductible plans cover eligible charges after your deductible at 80% in-Network and 50% non-Network until you meet your out-of-pocket maximum. A Prescription Drug Card* with a $100, $250 or $750 (respectively) per person, per calendar year deductible is included with this plan. -- Prescription Drug Card Copays are $5 Generic, $30 Preferred Brand Name and $50 Non-Preferred Brand Name

Combo Plan

Employer groups of 2 or more can offer any two plans to their firm. For example, an employer could offer the HDHP Member Choice 100 plan to all attorneys and a $1000 deductible plan to all non-attorneys.
