Disability Income Insurance

One of the best ways to make sure your income will be protected if you become disabled is to own an individual disability income policy. This policy non-cancelable coverage until the age of 70 unless you retire, cease to be actively engaged full time in your profession, or neglect to pay the premium. This means that you may renew your policy at the end of each term until you are age 70.

Eligible Applicants:

To apply you only need to meet the following criteria*:

• Under Age 50
• Optometric Profession under the age of 55
• Member of the Oklahoma Association of Optometric Physicans
• Work in your profession at least 30 hours per week

*Meeting this criteria doesn't imply the applicant will be approved,
applicant is subject to health underwriting

• Physician's fee for treatment for non-disabling injuries
• Pregnancy treated as any other illness
• Benefits payable regardless of other insurance
• $1,000 accidental death benefit, up to $100,000 available
• Pays 50% to 100% for accidental loss of limb(s)
• Premiums suspended after you receive benefits for 6 consecutive months
• Pays a minimum lump sum amount for specific fractures
• Survivor Insurance
• Receive benefits for mental, nervous disorders, alcoholism or drug addiction
